- To be completed by any prospective student-athlete that resides and/or attends high school outside the state of California upon first contact between the prospect and coach.
Name, Image and Likeness Disclosure Form - Click Here
- This form is required by law to be submitted to your institution's Athletics Director each time you engage in a promotional activity with your name, image, likeness, or athletic reputation for which you have been or will be compensated, regardless if such work is associated with the sport that you compete in (e.g., a member of the golf team has an endorsement contract as a professional surfer must still fill out this disclosure form).

- PRE-PARTICIPATION PHYSICAL FORM
- The pre-participation physical form must be signed by: MD, DO, NP, FNP, or PA-C. We do not accept physicals signed by DC. The pre-participation physical must be done annually and submitted to the athletic training staff prior to the first official day of practice.

Sports Ware Online is the way student athletes will sign and submit all medical releases, insurance documentation, emergency contact information ect. All documentation must be updated annually.
